TYPES OF SCHOOL VACCINATION CLINICS IMMUNOVAX PARTICIPATES IN: 1. "Walk-in" vaccination clinics. These clinics are typically held during early registrations for school, after school hours on a school day, or in the Summer before school begins in the Fall. 2. In-school vaccination clinics. These clinics are usually held during school hours, on campus. 3. Miscellaneous clinics. Include clinics held at health fairs, special school functions, or clinics designed for school district staff and employees. A TYPICAL PROTOCOL FOR IN-SCHOOL VACCINATION CLINICS: Although we can modify our procedures to accommodate any school district’s policies or
wishes, the following is a proven protocol that has been adopted by the majority of our districts
in the past: 1. School officials identify and tally the number of individuals needing each type of vaccination. 2. Immunovax personnel supply the appropriate type and number of forms needed to school
officials for distribution. 3. School officials then send these forms to the appropriate parents, along with a letter
explaining how the in-school vaccination clinic is to be conducted. 4. Parents send all completed forms and money (cash) to the school on or before the
clinic date. 5. Immunovax personnel clinically screen and vaccinate the children on the clinic date, at the school. Common Questions: 1.Do parents have to be present for children to receive vaccinations at school? No, as long as the correct paperwork is filled out completely and signed by a parent or guardian of the child receiving vaccinations. 2. Who gives the vaccinations? Only a pharmacist or nurse can administer vaccinations for
Immunovax. 3. Can my child receive vaccinations without completed paperwork? No, all paperwork must be
completed for us to screen patients and administer vaccinations. 4. Can I keep the advisory that is entitled, “What You Need To Know”? Yes, the parent should tear off
and keep this paper for future reference. It provides important information regarding the vaccination to be given. 5. Who do I make the check out to? Immunovax 6. Can I bring my other children to the school to receive vaccinations during the clinic? It is up
to the school officials holding the clinic. Most districts encourage participation by younger or older siblings.

against certain deadly diseases. Our records show that your child is in need of the following
vaccinations: On _______________ a mobile vaccination clinic (Immunovax) will visit our school and
administer immunizations during school hours. Vaccines at this clinic will be provided by the
Texas Department of Health’s Vaccines for Children Program. The cost will be $10 for children
receiving one shot and $15 for children receiving two or more shots. If you wish to have your
child vaccinated, please read and fill out the provided paperwork completely. Send it, along
with the appropriate fee (cash or check) to school on or before the clinic date. All paperwork
must be properly completed in order for us to administer your child’s vaccination(s). Check
with your school to determine if they will allow you to bring younger or older children to the clinic for
vaccinations. Although they are not yet legally required, hepatitis B vaccine for children 13 to 18
years of age will also be offered. In Harris, Galveston and certain other counties, hepatitis A vaccinations will also be offered under this program. These are deadly
diseases and you should seriously consider vaccinating your children against them. Since
millions of people become infected and thousands of people die annually from these two
diseases, you can imagine just how valuable a $10 or $15 investment in your child’s future can become.
